07/09/20 Update : Dr Elizabeth McCorkle COVID-19 Policies and Procedures


I am very excited to reopen my office but in order to do so safely we need to review several changes in the policies and procedures at Dr Elizabeth McCorkle.


First and foremost, my priority is maintaining your health and the health of others; therefore, we must minimize all physical contact and practice social distancing (staying 6 feet apart) whenever possible. So, while the following precautions may seem excessive, please realize that I am thinking of you and your safety. If you do not feel comfortable following this protocol, please let me know. 


The following precautions and changes to Dr Elizabeth McCorkle’s office procedures are based on strict CDC infection control guidelines which were developed to minimize the risk of SARS-COV-2 transmission to patients, healthcare workers, and others. As more information becomes available this protocol may be revised. Please read these carefully and contact Dr Elizabeth McCorkle with questions. You MUST FILL IN AND SUBMIT the form at the bottom of this page to secure your appointment.


Botox, facial filler, and Kybella injections are non-emergent and elective procedures. All patients are asked to thoughtfully consider the risks versus benefits for themselves. Patients in higher-risk groups for COVID-19, such as those over age 60 and those with compromised immune systems or lung and heart function, are encouraged to delay having these procedures. 

Please be responsible and caring of yourself and others. I will  frequently reassess these procedures and will not hesitate to close my office again if I feel that it is unsafe to continue operating.

  • Office Appointment Protocol Patient Screening For COVID-19:

  • New Patients (ONLY Patient who are NEW/Never been seen to Dr Elizabeth McCorkle):

    • New Patient Intake Form must be completed and submitted prior to your arrival. This may be found HERE or you may request that I email this to you.

    • Consents will be signed at the office.

      All Patients:

    • The day before your appointment please self-review and answer the Screening Questions  HERE. If any positive answers, please reschedule immediately

  • Day of your appointment: 

    • BRING A PEN:  AT MY OFFICE you will read and sign a Covid-19 Attestation Form and a COVID-19 Risk Informed Consent before being seen (I will have these forms at my office - you do not have to print these before your visit). 

    • SELF ASSESS: if you are not feeling well or have any of the symptoms listed in the screening questionnaire, please reschedule your visit

    • Check your temperature at home

    • Wash your face so that your skin has no makeup for your appointment

    • Wear a cloth face covering that covers your nose and mouth, regardless of symptoms. We will remove this at the time of treatment if needed

  • Arrival to my office:

    • Call/text me from your car  (541) 335 - 1928 

    • Once I have confirmed your arrival you may come to the front gate. Please continue to wear your face covering.

    • You will be asked the screening questions and your temperature will be taken with a touchless thermometer

    • You will clean your hands and complete the Covid-19 Attestation Form (see copy of this below)

    • Please arrive alone - no visitors!

    • Only carry your pen, reading glasses (if you need them), method of payment, and car keys

    • DO NOT BRING: purse, phone, extra clothing - VERY IMPORTANT

After Patient Screening:

If a patient reports not having symptoms and no fever is detected, they will be directed to my treatment area. Patients should still wear their face covering until directed otherwise and maintain a safe distance (at least 6 feet) whenever possible. Do NOT TOUCH ANYTHING


After Patient Appointment: Immediately after departure all surfaces within 6 feet of the patient will be thoroughly disinfected

  • Appointment times are scheduled with sufficient time between to allow thorough disinfection of office and to respect social distancing

  • All patients will be contacted 2 weeks after their visit to check on health status. Please respond to let me know if you have had ANY change in your health. 



  • ONLY Venmo, Cashapp, or Credit Cards

  • No Cash, checks or Care Credit

  • Brilliant Distinctions: Please arrange this before your appointment. I will NOT be able to process any credits on the day of your appointment



Doing My Part:

  • Observation of 6’ social distancing unless during direct patient treatment

  • Daily self screening for symptoms 

  • Temperature taken twice daily

  • Hands washed thoroughly between before each patient visit, again if needed during visit

  • Clean lab coat

  • Face mask at all times

  • Gloves, eye protection
