Dr Elizabeth McCorkle Office Policies and Procedures 

 

My office, which is located in my home, continues to follow careful cleaning policies.  I appreciate your understanding. As of April 24, 2023, Oregon no longer requires the wearing of a mask in medical settings. I will continue to wear a mask but do not require or ask that you do so.

Please read the following information and complete the form at the bottom of the page to confirm that you have read and understand my policies and procedures.

Please note:

• New Patients (ONLY Patients who are NEW/Never been seen by Dr Elizabeth McCorkle):

  • New Patient Intake Form must be completed and submitted prior to your arrival. I will email this to you once you complete the Online Agreement at the bottom of this page

  • Please check your spam folder if you do not see the Intake Form in your email folder once I text you that I have sent it

      All Patients:​​

• • Morning of appointment

    • BRING A PEN:  Better yet, bring two pens, to complete paperwork. I will not have extra pens available. Forget your pen? I have them available for you to purchase 

    • Wash your face so that your skin has no makeup for your appointment. Mascara is fine.

  • Arrival to my office:

    • Bring your pens, reading glasses (if you need them), method of payment, and car keys. You may bring a purse.
