Welcome to my practice! My office is located in my home, where I continue to follow strict cleaning practices to ensure your safety and comfort. Thank you for understanding! As of April 24, 2023, Oregon no longer requires masks in medical settings. While I’ll still wear a mask for your safety, you’re welcome to choose whether or not you’d like to wear one during your visit.

New Patients (First-Time Visits)

If you're a new patient, please complete and submit the New Patient Intake Form before your appointment. I’ll email the form to you after you complete the Online Agreement at the bottom of this page.
Tip: If you don't see it in your inbox, please check your spam folder just in case!

Morning of Your Appointment

Bring a Pen (or two!): Please bring your own pens for completing any paperwork. If you forget, I have pens available for purchase.

Wash Your Face: Please come with a clean face (makeup-free, except mascara). This helps me give you the best treatment possible!

Do NOT leave your car: Please text me from your car for instructions at (541) 335 - 1928.

What to bring: Your pens, reading glasses (if needed), payment method, and car keys. Feel free to bring your purse if you'd like!
