Dr Elizabeth McCorkle COVID-19 Policies and Procedures 


Please be thoughtful and read carefully.


My priority is to protect you, your health, and the health of others by minimizing physical contact and practicing social distancing (staying 6 feet apart) whenever possible while providing excellent care.

As more people are getting the COVID-19 vaccine and things are beginning to open up, it is very important to remember that there are people who are not vaccinated for many reasons ranging from personal choice to medical conditions that prohibit their obtaining the vaccine. What that means is that my office will continue for now to maintain very strict precautions and require you to do so as well when you are visiting for an appointment. I appreciate your understanding and adherence to the following policies and precautions.


The following precautions and changes to Dr Elizabeth McCorkle’s office procedures are based on strict CDC infection control guidelines which were developed to minimize the risk of COVID-19 transmission. As more information becomes available this protocol may be revised. Please read these carefully and contact Dr Elizabeth McCorkle with questions. You MUST FILL IN AND SUBMIT the form at the bottom of this page to secure your appointment.


Botox, facial filler, and Kybella injections are non-emergent and elective procedures. All patients are asked to thoughtfully consider the risks versus benefits for themselves. Patients in higher-risk groups for COVID-19, such as those over age 60 and those with compromised immune systems or lung and heart function, are encouraged to delay having these procedures. 

Please be responsible and caring of yourself and others. I will frequently reassess these procedures and will not hesitate to close my office if I feel that it is unsafe to continue operating.

  • Office Appointment Protocol Patient Screening For COVID-19:

  • New Patients (ONLY Patient who are NEW/Never been seen to Dr Elizabeth McCorkle):

    • New Patient Intake Form must be completed and submitted prior to your arrival. This may be found HERE or you may request that I email this to you.

    • Consents will be signed at the office so there is no reason to print these in advance

      All Patients:

    • The day before your appointment please self-review and answer the Screening Questions  HERE. If any positive answers, please reschedule immediately

  • Day of your appointment:

  • Prior to arrival

    • BRING A PEN:  Better yet, bring two pens. If you do not have a pen I can provide one for you at a cost of $5 per pen. You will have paperwork to fill in at my office so this is very important.

    • SELF ASSESS: if you are not feeling well or have any of the symptoms listed in the screening questionnaire, please reschedule your visit

    • Check your temperature at home

    • Wash your face so that your skin has no makeup for your appointment

    • Wear a mask that covers your nose and mouth. Do not remove your mask unless I indicate that it is ok to do so.

  • Arrival to my office:

    • DO NOT LEAVE YOUR CAR: Call/text me from your car  (541) 335 - 1928 

    • You will be asked the screening questions and your temperature will be taken with a touchless thermometer

    • You will clean your hands

    • Arrive alone - no visitors!

    • Only carry your pen, reading glasses (if you need them), method of payment, and car keys

    • DO NOT BRING: purse, phone, extra clothing - VERY IMPORTANT

After Patient Screening:

If a patient reports not having symptoms and no fever is detected, they will be directed to my treatment area. Patients will wear a mask until directed otherwise and maintain a safe distance (at least 6 feet) whenever possible. DO NOT TOUCH ANYTHING AT ALL


After Patient Appointment: 

  • All patients will be contacted 2 weeks after their visit to check on health status. Please respond to let me know if you have had ANY change in your health. By signing the Agreement below you are also committing to respond to this follow up.
